Add custom form fields to learn more about your clients in group classes, communities, privates and more!

Custom forms are useful for getting injury history, personal preferences, and client info specific to that one session. If you are looking to collect information about a client such as their height, general preferences, we recommend custom fields which store directly on the client's profile.

How to add a custom form

  1. Tap on Add New in upper right hand corner

  2. Add the following information

    • Form name

    • Show for these templates

    • Show for these pricing options

  3. Click Add New Field to enter questions and question types

    1. Small text

    2. Medium text

    3. Number

    4. Drop down with options

    5. Check box

    6. Client Shipping Address

    7. Client Phone Number

    8. Client Birthday

    9. Date

    10. Liability Waiver

    11. Signature: collect a signature on every form! πŸŽ‰

    12. Custom fields

5. Click add field to add a new question

6. Click on "Save changes" when completed

When forms are submitted you'll find them in the same tab as where they are built.
