Lead forms are digital forms that collect information from potential customers (leads) who express interest in your business, products, or services. Typically, they ask for details like a lead's name, email address, phone number, and specific preferences or needs. The information collected helps businesses qualify and follow up with leads in a personalized manner.

How to set up a Lead Form

  1. Go to Lead Management > Set up > Intake Forms

  2. Select New Form

  3. Enter the following information:

    1. Name: internal name for the form - this will not be shown to clients

    2. Title: name that the client will see

    3. Subtitle: A description of the form. The subtitle is displayed on the public form page and does not display on checkout pages.

    4. Redirect URL: the URL where users will be redirected after form submission

    5. Custom lifecycle stage: The stage the client will be put into after completing the form. If not selected, it will follow the default lead stage as defined in settings.

    6. Custom lead source: A custom lead source for this form. If left blank, it will default to 'form response'.

    7. Show for these templates (optional)

      1. Leave empty if you're using this for embedded forms or landing pages.

    8. Show for these pricing options and communities

      1. Leave empty if you're using this for embedded forms or landing pages.

    9. Add new field: A great way to customize the form to your business! These will be the questions that will be displayed on the form
